Default notification policy should post a comment about the failed action

When a PR stack run fails, there is no notification of the failure posted back to the pull request. The status check or pipeline is marked as failed, but I’m usually watching the PR comments or my email for a notification of the run status.

It would be great if the default notification policy were updated to post a comment about a failed run back to the PR.
